Travel Vaccinations

We have recently updated our travel advice clinic to make it easier for all those involved.  The clinic is for those seeking advice on vaccination prior to travel.  Please note only some are avaialble on the NHS so you may need to go to another travel clinic to get the vaccination. 

Please print off and complete the following document.

Travel Forms

The documents included are as follows

1 Letter advising our process

2 Travel Risk assessment (2 pages) to be completed by traveller

3 Consent Form

4 Travel Health Service sheet advising whats avaialble on NHS.

Once complete please hand in the forms a minimum of 6 weeks prior to travel. We are unable to accept forms if a minimum of 6 weeks has not been given.

The nurse will check over the forms and then you will be telephoned from the surgery to make an initial telephone appointment to discuss the travel arrangments. The appointment will be on a Thursday afternoon. For families travelling then one person should be given permission to discuss the travelling arrangments for all travellers.

The Nurse will advise during the appointment how things are progressed from there i.e. if you need to come back to have the vaccination done at the surgery. We do not stock the vaccines - these have to be ordered from the pharmacy.  

We hope you understand we only provide advice at this time for the smooth running of the surgery.
