New Patient Registration

keyboardstethThe doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice you will be asked to fill in a form for yourself and any other person registering with you. All new patients will also be asked to complete a patient questionnaire allowing us to provide medical care until your medical notes are transferred to us.

To register with us you will be asked for some identification.  Driving Licence/Passport are preferred I/D.  If you cannot provide I/D you can be introduced by someone who is currently registered at the practice.

We would also like to give you the opportunity to participate in an initial consultation with our practice nurse or healthcare assistant.  Please contact reception should you wish to make an appointment at this time.

If you have any chronic disease, such as asthma or diabetes, you will be invited within 6 months for a review.

Medical treatment is available from the date of registration.

If you move out of the practice area you will need to register with another practice. We can provide information about how to find a new practice. Please contact reception for further information.

You may print off a registration form and a new patient information form, fill it out both of these and bring it in with you on your first visit to the practice.
